Our work across population health analytics embeds the approach to improve the population’s health while reducing inequalities across several areas:
Disease surveillance and analytics
- Ageing and multimorbidity mean that population health (and illness) profiles vary more than ever. We use our cutting-edge modelling approaches to estimate health needs of populations and disease patterns in granular detail for today and tomorrow. We then work with healthcare providers to develop proportionate and targeted approaches to address the health needs in an equitable way and provide ongoing surveillance to monitor the impacts.
- With finite resources, decision makers want to invest in services and therapeutics with the biggest return on health. Our intervention modelling brings the choices and trade-offs to light. We estimate the health, economic and equity effects of a range of interventions, from therapeutics to public health measures, for individuals or whole populations, to allow comparisons in an interactive dashboard to inform decision makers.
Identifying inequalities within disease areas
- Inequalities have persisted across and within disease areas yet the scale of the challenge can make it difficult to know where to start. We identify the drivers and contributors to inequalities within disease areas, from disease onset, treatment pathway and access, to provide actionable insights for a range of stakeholders to address these challenges across the health system.
Redesigning care pathways according to patient health need
- Individuals living with just one chronic disease are becoming the exception rather than the norm. We map out clusters of conditions in specific populations, patient groups and individuals. This supports the redesign of care pathways to coalesce around the full health and prevention needs of patients, enabling more joined-up care and fewer clinic visits.

Multimorbidity, living with two or more conditions is one of the biggest challenges facing patients and health systems today. We leverage real world datasets and innovative approaches to generate evidence that identifies patient groups with the greatest unmet needs in a more holistic manner that reflects the drivers of ill health.
The majority of health systems use activity-based payments providers but this does not ensure patient and population health is at the heart of all decisions. We work with payers and providers to design and implement innovative reimbursement solutions that realign incentives to deliver value to patients and the wider health system.
We integrate a wealth of health and health-related datasets and develop bespoke interactive visualisations to make it accessible and rapid for clients to glean insights across a range of areas.
